I think one reason why it's difficult for shell providers to make it past their first year, is due to k-lines/g-lines/d-lines, abusive users, and packeting. To ensure you don't get blocked off of servers, you need to crack down on abusive users from the very start, and in doing so, you cut off the few people helping pay your monthly bill. To stop the packeting, you need to cut off the advertising accounts, and in doing so, you cut off the addition of countless users to your shellbox. If you don't stop the packeting, you end up pulling out a 2 month downtime notice while you relocate to a new colo facility, or have a new line provisioned from a different ISP, which is one of the worst things that can happen.
